One possibility on OSX is to use a terminal command:

(Of course, you may change ' 5 ' to whatever you like; The range seems to be between 0 and about 7.5 (?))

Is it normal for a computer to lag when sound on a video gets too loud, or when playing music loudly? I have a laptop running vista, 2GB Memory, 512 MB video card, 128 MB integrated sound card. Would upgrading the sound card fix this?

The sudden albeit small power drain from a loud sound might be a problem.
Wild guess: the noise is loud enough to make some automatic head parking is external speakers.

I don't know anything about Macs, but a grinding sound from a fan that happens at regular intervals, that is fixed by resting it sounds very much like heat is expanding a support, causing the fan to hit something else .

If your CPU is that hot, you really should remove the heatsink fan, re-apply some good thermal paste, and re-apply the heatsink... .
Your temps are not 'high' vis-a-vis how high those chips can stand, but they are extremely high compared to 'normal' operation .

Remove the old one and take it down to your nearest computer parts shop and get one the same size and rating and with the same plugs to replace it .
The cheapest solution is to buy a new fan.

The ToneBone BassBone preamp/DI actually was built for this purpose .
