Taken literally, Janeway broke that order taking this action.
Janeway was a little bit insane, (heck after all that happened to her, who wouldn't be)? But Admiral as long as they are nessesary to observe this directive.

I dont remember the name, but its a two parter. She basically single handedly, with gun rids the ship of nastys, very Rambo style.
