When a child is adopted, does their surname change to that of the adoptive parent? For example... John Smith adopts Adam Black. Does Adam become 'Adam Smith' or does he retain his birth-name?

Caveat- I am not an adoptive mom, but I am the mom children, and becoming dependent....
