You do not need to have it signed for an allergy, and you do need to tell the server a check list for equipment....
Mine is just a skin/rash variety of allergy that I know of - haven't tested it since I was 11 there is a medical form.

We just found out DD has a peanut allergy - I don't know what the bearing is on severity, but during the skin test it was over the size of a dime, so I'm calling it a significant allergy (not to say that others aren't, an allergy is an allergy I'm sure...

While you are there....get her tested for the high allergy food items.
They can give her a blood test nut allergies.
Yes...you can take her to an allergist and have her tested.
