Carnival has changed from formal to "elegant" nights....again, it's up to you how formally you wish to dress...
NCL has no "FORMAL' nights....they have a "dress up or not" night, so it's up to you.

If you put what cruise you are looking at somebody that has taken that cruise can tell you .
No only how many depending on length of cruise.

If it is ten day, it would usually be three formal nights.
If it is a seven day cruise, it should be two formal nights.
