Well, the most obvious question here is...do you witness ice dams forming? If so, then that's almost certainly due dams....
All great ways to get a roof leak from a ice dam.
No or not enough insulation, and no venting.

I think, and correct me if I'm wrong, that the dam was pretty much where you fished at the start, with a little venturing up each side, and not very far at that.
