I just wanted to mention that some airlines only give you one form per household. You require one per person. We are leaving soon on our trip and I will see if they will try that one on us again ;} this time I remember!

These were not able to be used....
There are two parts to the form, the immigration sided form they need to be all right :) Don't forget to fill out the departure record at the bottom.
It was our airlines responsibility to supply them..

Nogvmt - I have heard that US immigration is often chary of people who have green cards became a citizen.
Time we travel, whether on a cruise, and/or by air we are held-up at immigration/customs and asked revenue either.
