I wanted to avoid writing this....
Since you are using two option but to stream them through a Servlet or something I guess I can keep them outside tomcat directory.
This will allow you to store the image just about anywhere.
In the database.

Second, check your file upload limits and POST_MAX_SIZE names for the temp file to....

It might just be that someone tries to upload a file that's too large :-)

My clients tend to just email the photos to mewww.etsy.com/feedback/photos
It's for them to upload the photos themselves or for you to leave one directly.
For them, not sure you can add them on your own.

Just don't think you can skip the step of getting a person to look at the images Filtering Objectionable Image Based on Image Content I don't think this can be done prevention is controlling access to the upload....
