I think you need further evaluation of your liver, preferably by a liver specialist ( Heptologist)
Have you been tested for hepatitis b and c?
Part of the problem is using the ER for this, you really need a doctor who can look into it more thoroughly....

The pain....
Two weeks ago I woke up with severe pain in my right shoulder and down my arm.
Chest to the center/left side of my chest, right where my heart is, but after getting and EKG the diagnosis is!!.

I get this from time to time and have anxiety. I've even had a nuclear Nuclear ventriculography stress test . . . results normal. I also get the pains in my left and right chest and lower abdomen, but it always goes away. Eat light until it passes.
