Scrape out the old thin set that held it in clean up the area where it came out and use more thin set to adhere it back into the space. .

I live in a house that was built in the 50's. Our main floor bathroom is one of those pink and black tile bathrooms that we have never re-done (on the list) The toilet paper holder is tile, built into the wall at the side of the toilet.
My kids understand...

I can't get one of the mounts (the full decorative mount, not just the metal mount behind that) off the wall. The little screw at the bottom will not come out. I tried using one of those acre removal bits with my drill/driver, but that wouldn't do it....

My toilet paper roll is actually on the floor because the holder on the wall fell off. BUT if it were still there it would be in the overhand position.
