From past forum posts and answers from adsense, it seems yes that is definitively against. But I wouldn't be surprised if they don't enforce it as much as images above ads or labelling the ads. source

When your posting hit HTML once your done and post the code in or around the text. If you don't like the ads in the content get a plugin and post the code in that. source

I don't know about anyone moving to other ad providers. What I do know is that AdSense clicks are worth less and less by the day, which will inevitably start to get webmasters searching for monetizing alternatives. If this happens en masse, then AdSense is gone... I surely hope Google can be nice enough to increase the click values, the way it is now is just not going to work for very long. source

I am just presenting a sample scenario, I am neither A nor B. But this is a widely open loop hole, as there seems to be no protection outright, A has to have knowledge about server logs, and the server logs only show the URLs on your website, not the adsense URLs because they are included from javascript dynamically - very difficult to prove to Google. source
