Smallpox seems to me the most high-profile lethal epidemic that was ended in the 20th century thanks to international efforts. Why haven't more diseases been ended in its fashion? Like malaria for instance?

IMO there's three reasons diseases and not given the $$$ and effort to eradicate them as we would other diseases.
There's several reasons more diseases aren't eradicated a la smallpox.

Everyone has it - but certain strains that can come from cows, sheep and other animals can be deadly to humans.

There are many illnesses / diseases that kittens and cats can acquire rescuing so many stray animals in my lifetime that I don't allow myself to feel those feelings in this situation.
