Could limit the input capture to SMTP or POP3 and it would capture all the data transmitted over those being done for a single domain you might consider just signing up for email archiving services the system in question.
I have a number of clients in the heath care industry and when I've pitched them Google Apps they request information regarding HIPAA compliance. Does anyone have more specific HIPAA knowledge or experience answering these questions?
