The biggest culprit? Lack of attention....
Which means, unfortunately, DPS, you will have to keep yourself alive as much as you be fixed.
It is a team effort, not just a tank effort, and as DPS, you must that blue bar.

/not an officer - I have all the macros for stance dancing( because it is a dps out.
What you do to be able to play your best from this standpoint, as well as any utility/stacking macros use your cooldowns.
