Soiji - I just to say to put your adoption birth....
If I had gotten a passport prior to this, there would have been no problem I guess.
Many people do not have their actual birth certificates since.
Adoption papers should be fine.

I went to get my passport done today and was total that i would need a change of name certificate(which is $150 and takes 8-10 weeks) as when i got married i hypenated my maiden name and my husbands last name.

For the purpose of the passport? You don't need a change of name certificate.
