We have just been asked by our CO to provide evidence of my partners name change as her middle is included on her passport and not on her birth certificate. She has never had her name officially changed by depol so I don't know how this has happened, ...

After DD was born we meant to mail in the request for her birth certificate. We had to get a certified check to send with the request and just totally forgot about it. We are flying with DD on Friday and I was told we should have it with us just in case...

After we booked our trip we realized that I have had my passport before being married. It has my maiden name and all my other ID's have my current married name....Our travel agent has changed my name...

You do not need your drivers license to match if you are using your passport children's father, and they all have the same last name.

I think they can only certified/attestation to a copy of original document /birth certificate.

With a valid passport, there is no reason to be travelling with your birth certificate.

I have heard that you don't need a passport to travel to Mexico from the US, as long as you have an official birth certificate. Will I have trouble if I do this? I was just married and already had my name changed on my license. I am also bringing my marriage...
