Of course, because the pprint macro exists, it's not strictly necessary for you to follow standard ... .
Having ONE TRUE FORMAT is always a good thing for code, and the pprint macro gives you that format built into the language.

Calibre handles most of the places where I have used a stylesheet... .
The way I do it is to make the ePub in valid XHTML files with an external style sheet, content.opf and toc.ncx files, package them up into a valid ePub file, and use calibre to convert to mobi .

You could set the position to fixed and set the top property to 940px .
I hope I understand your question correctly, but if you contain the pagination in an element, say a div with id="pagination" .
