I see from old threads that wilderness medicine seems to be a fellowship after EM residency. So what sort of "jobs" does this unlock and how hard these to obtain?
I am also confused...if you are far from a clinic and have no labs, no imaging, very few...

They may get a small wilderness medicine job on the side ranging from working at a ski resort to being a physician at Everest base camp....
Almost all if not most wilderness medicine physicians still spend most of their time working in an ED.

So I had a guy come in with cirrhosis w/ known varices s/p banding/ligation a few days ago - had a few syncopal events prior to presentation.

We are almost out of etomidate, midazolam and lorazepam. Barbs? Nah, haven't had any in years. This basically leaves propofol, ketamine and brutane. I'm actually considering lower induction doses of propofol with a phenylephrine chaser for my septic patients...

I just read in the latest NRMP program director survey that EM programs put equal eight on step 2 scores as they do on step 1 scores (3.8 and 3.8)
Everyone keeps telling me that step 1 is so much more important. One EM attending even told...

I would take the NRMP survey to be more correct than your n=1 .
Attendings do not necessarily equal program directors.
