Fill all the necessary fields and affix the photocopy of the FIR along with the application .
Then apply for the new passport as you did before.
They will provide you a Non Cognizable FIR copy.
That youve lost the passport.

I think you will have to get a new passport going to an embassy or consulate and they can actually add on some pages to a passport if it still has they will do it without it being....
