Valid visa is not required to stay in U.S.
Visa is only an entry doucment to U.S.
The I94 with out leaving the USA? The H4 visa could have issued for a lesser duration than the approval.

It gets complicated as many states have waiting period for marriage license to be issued .
Cannot be on H4 status once the marriage has ended.
