This book is a little outdated, but is very useful in writing you own form. Most of the forms on the disk can't be customized using newer word processing software.

I bought this book and its utter waste of paper and money. There are much better forms available for free to be downloaded from the internet. This book is completely useless for anyone who lives outside USA. I think Americans are smart enough to google...
